Insurance Requirements and Information


The Subcontractor shall maintain minimum insurance coverage as follows:

General Liability
General Aggregate                                                                $2,000,000

Automobile Liability

Umbrella (Required for abatement, electrical, mechanical,
roofing and elevator subcontractors)      $1,000,000             
 
Worker’s Compensation and Employer’s Liability
 
Bodily Injury by Accident - Each Accident                               $1,000,000
Bodily Injury by Disease - Policy Limit                                    $1,000,000
Bodily Injury by Disease - Each Employee                              $1,000,000

 

The Subcontractor will request its insurance company to provide an original Certificate of Insurance within ten (10) days of issuance of this Subcontractor Agreement stating the above minimum limits and shall provide the following endorsements:

 

The Subcontractor shall provide General Liability Insurance written on the comprehensive form of the policy, which does not exclude subcontractor's operations. Said insurance shall cover damages related to current and future claims based on incidents occurring during the performance of work under this contract. The insurer and policy provision proposed by the Subcontractor are subject to the prior written approval of ALLIED. Subcontractor shall furnish to ALLIED for approval one copy of said insurance policy, certified by the issuing insurance company.

 

Allied Industries, Inc., and OWNER shall be listed as additional insured for Workers Compensation, General Liability, Automobile Liability, and Excess Liability.

Allied industries, Inc., shall be listed as “Certificate Holder” at the following address:

North Hollywood, CA 91605
 
The certificate shall include the following cancellation statement:
 
Cancellation or any material change in the policies adversely affecting the interest of the Owner in such insurance shall not be effective for such period as may be prescribed by the laws of the state in which this contract is to be performed and no event less than thirty (30) days written notice to ALLIED.
 
Subcontractor aggress to name the Contractor, Allied Industries, Inc., the Owner's Representative, the Owner,  and their employees and agents, all as additional insured on all workers compensation, general liability and automobile insurance policies.
 
Certificates shall be issued specifically for the following and this description should appear on the face of the certificates:

Please Delete in the “Cancellation” the wording as follows:
 
            “Endeavor”
 
“But failure to do so shall impose no obligation of liability of any kind upon the insurer, its agents, or representatives.”
